help using a da polisher

Hey Lump, Can you make any comments why on a DAS6 with a 3m Backing plate - the backing plate flew off with all the thread ground down. When I fitted it I wound it on 1 spin and then span it down, so I know it wasnt cross threaded.
The Pad also fell apart eventually but I think this was where it got stuck to the 3m tape which pulled small bits off weakening the structure of the pad.
