Default Handbrake failure - a known fault?

I was walking into my garage this evening when I heard a clunk and turned round to see my wife's V6 CDTi Vectra Estate rolling down the drive. By the time I got to the front of the car the near side rear bumper had struck a tree! When I looked inside the car I noticed that the hand brake was completely flat i.e. off, the doors were locked and the alarm was blaring out

As much as I might have suspected "operator error" this was impossible as my wife had arrived home 3 hours earlier and the car was parked on a slight incline.

Has anyone had this happen to them or heard this before?

What's Vauxhall's likely stance going to be?

I have no confidence in mine even though its never failed just dropped a click as if the ratchet hasn't seated properly. I always park it in gear.
