As you have a 2004 vehicle, this could be either a full CAN or half CAN radio.
If it is full CAN, then the radio code should be the same as the vehicle security code.
If it is a half CAN radio, then it is the radio type / code number that you input.

just checking you are entering code correctly
press AS and turn the the stereo on while AS is still pressed in, when you see - - - - take your finger off AS, say your code was 4356 press number 1 4 times, number 2 3 times , you get the idea once you have entered code keep your finger on AS youll hear a beep and it will work

I see from your pass that the following details are probably correct
Radio type = CDR2005
Radio Code = 7481
Radio serial number = 32018483.
Suppose the first thing I would do is check that the serial number on the card, matches the serial number on the white sticker on the head-unit casing.
If the serial number is correct, then the radio code should be 7481.
I have heard of some instances where the CDR has thrown a wobbler, and will not allow itself to be programmed..
This shows itself by not displaying the page with the dashes on..
such as " CODE - - - - "
In the worst scenarios, the head-unit has been faulty and could not be programmed or reset.
If it is showing " 7 S A F E", then it is going through a wait period. (7 represents the remaining opportunities to input the code).. you would need to leave the ignition on until this page clears. After 3 incorrect entries, this wait period could be approx 15 to 20 minutes.

Attempt 1 - wait 10 secs. (Display shows 10 SAFE)
Attempt 2 - wait 10 secs. (Display shows 9 SAFE)
Attempt 3 - wait 10 mins. (Display shows 8 SAFE)
Attempt 4 - wait 20 mins. (Display shows 7 SAFE)
Attempt 5 - wait 40 mins. (Display shows 6 SAFE)
Attempt 6 - wait 80 mins. (1 hr 20 mins) (Display shows 5 SAFE)
Attempt 7 - wait 160 mins. (2 hrs 40 mins) (Display shows 4 SAFE)
Attempt 8 - wait 320 mins. (5 hrs 20 mins) (Display shows 3 SAFE)
Attempt 9 - wait 640 mins. (10 hrs 40 mins) (Display shows 2 SAFE)
Attempt 10 - landfill. (Display shows 1 SAFE) ps, Re Landfill comment. Not really, but you would have to seek expert help to get it unlocked.
And you have to leave the stereo switched on (and key in the ignition) until the wait time is over.
