Mine was like that. (mate who borrowed the car then tried to screw it back on when it popped off.... Which basically removed any thread left on the inside of the knob) Unfortunately there isn't a GM part number JUST for the gear knob. So you have to buy the whole stick, I ended up resorting to an irmscher knob.
Purely because I didn't want to risk fouling the reverse spring with glue that might seep down when trying to stick it back together.

Best thing to do is remove it by pulling it up sharply , make sure you don't lose the spring underneath it
Cut a small piece of cloth from say an old pillow case about 1cm x 1 cm
Place the cloth on top of the stick piece
    Replace the spring
Place the gear knob back on the stick
Then give it a sharp smack down wards with your palm
If that doesn't locate it properly them cover the top of the gear knob in an old towel and tap gently with a plastic or rubber mallet
    Hope this helps

Some people will glue them back on with a suitable liquid metal epoxy but more often than not, others will go for a Richbrook or Irmscher knob.

The entire lever (as also mentioned above) was replaced in my Vectra before I bought it for this reason, the result was a lovely tight gearshift patterns

Yep, the gear knob is a disaster. I used some cloths, matches, tissues and finally one mechanic put some glue underneath. After one year it starts to come off again and I still have no concrete idea...

Pull it up sharply, but as MarkTaylor said watch the spring as it does fly out, then just glue it back down, happened on mine a good year ago, was gonna get a six speed one from Irmscher but said they don't make em any more, hasn't come off since HTH !!!!!!!
